The Hounslow Integrated Care Partnership has allocated £50,000 for distribution as part of the Second Round of the Thriving Communities Fund to support
projects which help deliver against its key priority areas. For the ICP Vision slide pack click here
• Public Health and Prevention
• Mental Health
• Frailty
We would particularly welcome applications that address the following:
• Improve equality of access to preventative care services, particularly for BAME and other ‘hard to reach’
communities
• Increasing opportunities for Self-Care and the self-management of conditions in particular diabetes, respiratory
illnesses and cardiovascular disease
• Supporting people to live healthily in a safe environment, for example reducing incidences of falls within the borough
For ICP related projects, consideration will be given to proposals that have a review implication beyond the life of the one
year grant. However, applicants will need to be able to demonstrate how they would continue to fund this once the grant
finding ends. Small capital bids that support the delivery of the ICP priorities will be considered. Typically, proposals should
be up to a maximum of £10,000 per project. Proposals should focus on:
• Reducing inequalities
• Improving access to services
• Focus on maximum population coverage
• Meet the ICP priorities
• Achieve tangible improvement in Self Care and self-management
