Hounslow Cares Together
Services have been busy over the summer to develop the new model for care for Hounslow called Hounslow Cares Together. The twelve-month ‘test and learn’ will involve five GP practices across Hounslow Health Primary Care Network. Staff from a range of services have been selected to be part of the new multi-disciplinary team, and they have been undertaking training and building the systems needed to deliver integrated, proactive, and holistic care to residents.
The Hounslow Borough Based Partnership will be attending the September meeting of the Hounslow Health and Social Care Forum to update you on Hounslow Cares Together and how it can work with local voluntary sector groups delivering invaluable services to residents.
